What is Trichophagia?
Trichophagia is a compulsive behavior where a person repeatedly eats their own hair. This action is often linked to a related condition called trichotillomania, which is the urge to pull hair out. Most people miss this: the swallowed hair can form a dense, tangled mass in the stomach called a trichobezoar, which often requires surgical removal.
Think of it like a mental hiccup your brain can’t control, leading to a physical habit that damages both your hair and your body. It’s far more than just a bad habit; it’s a body-focused repetitive behavior that signals deeper distress.

Trichophagia and the Hidden Stomach Hazard
The science is startling but simple. Human hair is made of keratin, a protein our stomachs cannot digest. Think of keratin like a very tough, plastic-like thread that your gut can’t break down.
When you swallow hair, it doesn’t just pass through. It accumulates in the stomach, matting together with mucus and food. I’ve seen patients who required surgery because this hairball, or trichobezoar, grew so large it caused a blockage.
This mass can lead to pain, nausea, weight loss, and even life-threatening intestinal obstructions. It’s a severe physical consequence of a psychological compulsion.
Why Trichophagia Targets Your Hair Health
This condition creates a double assault. First, the pulling damages the hair follicle itself, potentially leading to traction alopecia or scarring over time. I always check for broken hairs of varying lengths and patchy loss, which are classic signs.
Second, the act of chewing the hair shaft causes extreme mechanical damage. It frays the cuticle, the hair’s protective outer layer, and snaps the cortex, its inner strength core. The hair you see is often severely weakened long before it’s pulled out.
This damage pattern is distinct. It’s not the even thinning of genetic hair loss or the diffuse shedding of a health issue. The destruction is focused and self-inflicted, following the patient’s accessible “pulling zones.”
The Trichophagia and Trichotillomania Link
Trichophagia almost never exists alone. It’s the “second step” in the cycle for many with trichotillomania. The pull provides a sense of relief or satisfaction, and then eating the hair root or shaft completes the ritual.
Think of it like a circuit that gets stuck. The brain creates an overwhelming urge to pull, the act brings temporary calm, and consuming the hair feels like a necessary conclusion to the cycle. Breaking one part of the cycle is key to treating the whole.
In my practice, I find that treating the pulling urge directly, often with behavioral therapy, automatically reduces the eating behavior. You can’t eat what you haven’t pulled.
Spotting the Signs of Trichophagia
The hair clues are very specific. Look for uneven, patchy hair loss, especially in easily reached areas like the crown, front hairline, or eyebrows. You’ll see broken hairs of different lengths, not just shedding from the root.
Beyond the scalp, check for thinning or missing eyelashes and eyebrows. People often don’t realize they’re pulling from multiple sites. I also ask about stomach aches, nausea, or a feeling of fullness, which can be early signs of a forming bezoar.
The most telling sign is often secretive behavior. Patients may eat hair privately, hide the resulting bald patches with hats or styles, and feel deep shame about it. They usually know it’s a problem but feel powerless to stop.

The biggest hurdle isn’t the hair; it’s the shame. People with trichophagia often wait years to seek help because they believe it’s just a “gross habit” they should control. I make it clear from the first visit: this is a recognized health condition, not a personal failing.
My most effective approach is a team one. As a trichologist, I manage the scalp health and hair regrowth strategies. But I always refer to a therapist specializing in cognitive behavioral therapy (CBT) or habit reversal training to tackle the core compulsion.
