Traction alopecia is a type of hair loss that happens when you pull on your hair too hard for too long. It’s a slow process where constant tugging damages the hair roots, making them stop growing new hair.

How Traction Alopecia Chokes Your Hair Follicles
Your hair grows from a tiny organ in your scalp called a follicle. This follicle needs good blood flow and a healthy environment.

Constant pulling creates inflammation and cuts off that vital circulation. Think of your hair follicle like a drinking straw being pinched shut—nothing can flow through.
I see this when patients come in with thinning edges from years of tight ponytails. The follicles are literally being strangled. Over time, this inflammation causes permanent scarring, so the follicle can never produce hair again.
The Sneaky Styles That Cause Traction Alopecia
It’s not just about tightness; it’s about consistent tension on the same spots. The usual suspects are tight ponytails, buns, braids, cornrows, and hair extensions.
Any style that pulls your hair back firmly from your face is a risk. Weaves and clip-in extensions that are anchored to small sections of your natural hair are major culprits because they concentrate weight.
In my clinic, I find that combining tight styles with chemical relaxers is a perfect storm for irreversible loss. The relaxer weakens the hair shaft, and the tension finishes the job.
Spotting Traction Alopecia Before It’s Too Late
The earliest sign is not hair loss—it’s discomfort. A sore, tender scalp, itching, and those small red bumps (folliculitis) along your hairline are your first red flags.
Next, you’ll see short, broken hairs around the temples and forehead. Finally, the hairline starts to recede, or you’ll see thinning patches where the tension is greatest. The hair loss in traction alopecia often follows a very distinct pattern, outlining where the style pulled.
About 70% of my clients ignore the itching phase. They only book an appointment once they see visible skin, which often means the damage is already advanced.
Healing Traction Alopecia: What Actually Works
The single most effective treatment is to remove the source of tension immediately. This is non-negotiable. You must give your follicles a complete break from any pulling.
Switch to loose, low-manipulation styles. Think loose braids, buns at the nape of your neck, or wearing your hair down. I often recommend a period of wearing protective styles that don’t pull on your roots, like loose twists.
Topical treatments like minoxidil can help stimulate follicles that are dormant but not yet scarred. For inflammation, a dermatologist might prescribe a topical steroid. However, if scarring has set in, these treatments won’t regrow hair.
When Traction Alopecia Leaves Permanent Scars
This is the hardest truth for my patients to hear. Once the follicle is scarred (a state called cicatricial alopecia), it’s gone for good. The body replaces it with fibrous tissue.
Think of it like a deep cut on your skin that forms a scar—no hair grows on a scar. A scalp biopsy can confirm if this scarring has occurred. In these cases, the goal shifts from regrowth to preventing further loss and exploring cosmetic coverage.
Options like scalp micropigmentation (SMP) or hair transplantation (if the donor area is healthy) become the focus. I never recommend a transplant until all active tension and inflammation have stopped for at least a year.
From My Experience
The most common mistake I see is people blaming “weak hair” or genetics for their receding edges. They don’t connect it to the high, tight ponytail they wear to the gym every single day. Your hair’s strength is irrelevant when the force is strong enough to damage the root itself.
I have a simple test I ask clients to do. After taking down a style, gently run your finger along your hairline. If you feel soreness or tingling, that style is causing damaging tension. That sensation is the inflammation starting.
My proprietary insight is to watch the “baby hairs.” If they’re not growing past a certain length and keep breaking off in the same spot, that’s not your natural growth pattern. It’s a classic sign of chronic, low-grade traction damaging the hairs as they try to emerge. Catching it at this stage gives you the best chance to reverse it completely.
