What is Tinea Capitis?

Tinea capitis is a contagious fungal infection that attacks the hair shaft and scalp skin. It’s often called scalp ringworm, even though no actual worm is involved. This infection invades the hair follicle, the tiny pocket where each hair grows from.

Most people miss this: The fungus doesn’t just sit on the surface. It actually grows inside the hair shaft itself, making it brittle and easy to break. This is why you see those characteristic “black dots” of broken hair on the scalp.

How Tinea Capitis Hijacks Your Hair Follicle

The fungus releases enzymes that digest keratin, the core protein your hair is made of. Think of it like termites eating away at the wooden structure of a house. This weakens the hair from the inside out.

Your immune system tries to fight back, causing inflammation. I see this when patients come in with sudden, patchy hair loss and a scaly, angry scalp. The broken hairs left behind feel like coarse stubble.

The Surprising Contagion of Tinea Capitis

This fungus spreads through shared objects, not just head-to-head contact. Combs, hats, pillowcases, and even headphones can carry the spores. It’s most common in children, but adults are not immune.

In my clinic, I’ve traced outbreaks back to a single shared hairbrush at a sleepover. The spores are tough and can live on surfaces for a long time. This is why isolation of personal items is my first rule for affected households.

When Tinea Capitis Mimics Other Hair Problems

The symptoms can look like severe dandruff, psoriasis, or even an autoimmune hair loss condition called alopecia areata. A key difference is the broken hairs and sometimes swollen, pus-filled areas called kerions.

I always use a special ultraviolet light called a Wood’s lamp for an initial check. Some types of tinea capitis will glow a faint green. However, the only sure diagnosis is a microscopic exam or culture from a scalp scraping.

Why Treating Tinea Capitis Requires More Than Shampoo

Because the fungus lives deep in the follicle, topical creams and medicated shampoos alone usually fail. They can’t reach the infection’s root. Oral antifungal medication is the standard, necessary treatment.

Think of it like treating a tree with a root disease. Spraying the leaves won’t work; you need a systemic treatment that travels through the whole plant. The oral medication circulates in your blood and reaches the follicle’s base.

Medicated shampoos containing ketoconazole or selenium sulfide are used as a helpful sidekick. They reduce spore shedding and help prevent spread to others, but they don’t cure the infection alone. I tell my patients they are for public health, not primary treatment.

From My Experience

The biggest mistake I see is stopping treatment too early. The oral medication must be taken for weeks, often after all visible signs are gone. This ensures every last spore is eradicated to prevent a roaring comeback.

I also insist on a household check. Treating one child while a sibling is an asymptomatic carrier guarantees a cycle of reinfection. Sometimes, everyone in close contact needs to use the medicated shampoo for a short period.

Finally, patience is key. Hair regrowth in the bald patches only begins after the inflammation fully quiets down. It can take several months to see a full recovery. Nourishing the scalp and avoiding tension on new growth is crucial during this phase.
