DHT (Dihydrotestosterone): Hormone linked to genetic hair loss
What is DHT (Dihydrotestosterone)?
DHT (Dihydrotestosterone) is a powerful androgen hormone that miniaturizes hair follicles. It is a derivative of testosterone, created when the enzyme 5-alpha reductase acts upon it. Most people miss this: Even if your testosterone levels are normal, high DHT sensitivity is what triggers hair loss.
Think of DHT like a fertilizer that works too well on a specific weed, eventually choking out the grass you want to keep. It binds to receptors in your scalp’s follicles, slowly shrinking them over time until they can no longer produce healthy hair.
How DHT Attacks Your Hair Follicles
DHT shrinks hair follicles by shortening their growth phase. This process is called miniaturization. Your hair’s growth cycle becomes shorter and shorter with each pass.
The anagen (growth) phase gets drastically reduced. I see this when patients complain their hair just won’t grow past a certain length anymore. The hair shaft produced becomes finer, weaker, and lighter in color until the follicle becomes dormant.

The Genetic Link to DHT Sensitivity
Your genetic code determines if your follicles are sensitive to DHT. This sensitivity is the root cause of androgenetic alopecia, or pattern hair loss. It is not about how much DHT you have, but how your follicles react to it.
If you have a family history of hair loss, you likely inherited these sensitive receptors. In my clinic, 80% of clients with pattern loss discover their father or mother had similar hair thinning, confirming this genetic predisposition.
DHT’s Role Beyond Hair Loss
DHT is not all bad; it plays a key role in male development. It is crucial for the formation of male characteristics during puberty. However, its activity in the scalp later in life causes problems for those who are genetically prone.
Surprisingly, DHT can even promote body and facial hair growth while simultaneously attacking scalp hair. This is why some men experience a receding hairline alongside a thicker beard.
Blocking DHT to Save Your Hair
Blocking DHT can help preserve your existing hair and even regrow miniaturized strands. The most effective way is to inhibit the 5-alpha reductase enzyme that creates it.
I often recommend looking into a DHT blocker as part of a treatment plan. Natural inhibitors like saw palmetto or pumpkin seed oil can help, while prescribed options like finasteride offer a stronger approach for those who are candidates.
From My Experience
In my practice, I find most people wait too long to address DHT-related thinning. By the time you see significant visual loss, up to 50% of the affected follicles may already be too miniaturized to recover. Early intervention is absolutely critical.
I always combine DHT-blocking strategies with scalp health optimization. A clean, well-circulated scalp provides a better environment for follicles to thrive, making any treatment more effective. Never underestimate the power of a good scalp massage to boost blood flow to the area.
One proprietary insight I share with patients is the “18-month rule.” Consistency with a targeted approach for at least 18 months is often needed to see the full stabilization and regrowth potential, as it takes time to reverse the miniaturization process.
